ITEMS REQUIRED FOR APPLICATION FOR ADMISSION

MASTER OF BUSINESS ADMINISTRATION
FOR PHYSICIANS & DENTISTS

Please read the following instructions carefully when completing your application package.  After completing all the items required, send them to the address listed below. Applications cannot be processed until all information is complete.

CHECK LIST FOR ITEMS REQUESTED BY THE COLLEGE OF BUSINESS ADMINISTRATION:

o GRADUATE SCHOOL APPLICATION FORM – Please note that you do not need to fill in the section of the application pertaining to the computation of undergraduate and graduate GPA.  Also note that we have already filled in answers to some of the questions on the application.  ALL FORMS NEED TO BE PRINTED OUT,  FILLED IN AND MAILED BACK.

o ESSAY - Please type or print a one page essay regarding why you want to be part of the MBA program and what you hope to gain from the program. 

o RESUME - Attach a current resume indicating your employment and education history.  Please specify all of your management experience and number of employees supervised.  Also include your cell phone or evening phone number, e-mail address, and fax number.  Additionally, please include the following information regarding your medical practice history:

1.         Board certifications by specialty and date, and/or date when residency was completed and specialty.

2.         States where you are currently certified.

3.         Number of years of clinical practice (excluding residencies).

o APPLICATION FEE - A nonrefundable application fee of $50 will be required for all US citizens and permanent residents of the United States.  Checks should be made payable to Texas Tech University

o ADVANCED PROGRAM FEE – An advanced program fee of $1,000 is required upon application to the program. The advance program fee will be refunded if for any reason the program cannot be offered, or if an applicant is not accepted into the program. Because of space limitations, the advanced program fee will not be refunded after August 25th, 2007.  If you would like to pay by credit card please complete the credit card information on the bottom of the Program Fee Schedule and return the form with the rest of your application materials.  If paying by check please make checks payable to Texas Tech University.

APPLICATION DEADLINE – Application materials are due August 20th, 2007.  Late applications will be considered after the deadline; however, there is limited space available.

NOTIFICATION OF ADMISSIONS DECISIONS - Applicants will be notified on the decision regarding their admission soon after the receipt of all their application materials.
